The MGMini is Medical Guardian’s small mobile pendant, and it does a lot of things well — lightning-fast response times, a 5-day battery that’s genuinely rare for a device this size, and free caregiver tracking. David wore it under his shirt for an entire work week and nobody noticed it was there.
But we have to be upfront about the big problem: its fall detection is terrible. In our testing, it caught only 1 out of 10 simulated falls. One. If you’re buying this specifically for fall detection, stop and look at the MGMini Lite instead — same brand, bracelet form factor, and it caught 8 out of 10. The naming is confusing (thanks, Medical Guardian), but the performance difference is massive.
Editor’s Rating: 4.2 / 5
Key Findings
- In 10 test calls, the MGMini connected to a live agent in an average of 8 seconds
- Battery lasted approximately 4 to 5 days on a single charge — the longest of any Medical Guardian mobile device
- Fall detection detected only 1 of 10 simulated falls — too unreliable for users who need this feature
- Caregiver tracking through the MyGuardian app is included free (unlike other Medical Guardian systems, which charge $2.99/month)
Pros and Cons
Pros
- Among the fastest response times we’ve tested at any company
- Multi-day battery means less frequent charging
- Small, pendant-style design that fits discreetly under clothing
- Free caregiver app access with location tracking and emergency notifications
- IP67 rating makes it safe to wear in the shower
Cons
- Fall detection accuracy is very poor — do not rely on it
- Costs more per month than comparable devices like the Bay Alarm Medical SOS Micro ($39.95 vs. $29.95)
- 4G cellular only — no Wi-Fi fallback

Testing the MGMini
Setup
The MGMini arrived with a charging cradle, charging brick, USB cable, nonadjustable lanyard, adjustable lanyard, and belt clip. Setup consisted of placing it on the cradle to charge — approximately four hours to full. No activation fee was charged.
Emergency Response
Placing a test call required pressing the help button for two seconds. Across 10 test calls, the average time to reach a live agent was 8 seconds. GPS tracking was accurate throughout: agents correctly reported our outdoor location by cross streets and address on every call.
Fall Detection
The MGMini’s fall detection performance was the weakest result we recorded across all devices tested. In 10 simulated falls, the device detected just 1. This is well below acceptable for users who depend on automatic fall alerts.
The MGMini Lite — Medical Guardian’s bracelet-style device — uses the same base hardware but in a wrist-worn form factor, and detected 8 of 10 falls in our testing. The wrist position appears to make a substantial difference in fall detection accuracy.
Caregiver App
The MyGuardian app provides real-time location tracking, battery monitoring, and emergency call notifications. Unlike other Medical Guardian systems, caregiver tracking is included at no extra cost with the MGMini. The app interface was easy to navigate, and location data updated reliably during our testing.

Choose the MGMini if you want lower monthly costs and better battery life and do not need fall detection. Choose the MGMini Lite if fall detection is a priority.
Final Verdict
The MGMini delivers Medical Guardian’s signature fast response times in a small, comfortable form factor with a long battery life. Those are real strengths. The poor fall detection performance is a serious limitation that disqualifies it for anyone who specifically needs that feature.
For users who want a mobile pendant without fall detection, the MGMini is solid. For fall detection, look at the MGMini Lite or Bay Alarm Medical’s SOS Micro.
