Medical Guardian Costs, Plans, and Systems 2026

Medical Alert Pendant

If response speed is the thing that matters most to you, Medical Guardian is the company to look at. In our testing, their monitoring agents answered in an average of 8 seconds — and we’ve tested over 15 brands. Nobody else came close to that number.

Medical Guardian also has more device options than any other company we cover: five different systems from a basic landline setup to a full smartwatch. Our team tested all five over the course of several months. David’s favorite was the MGMini pendant, mostly because its 5-day battery meant he could forget about charging it for almost a week. Sarah preferred the MGMini Lite bracelet for its fall detection accuracy.

The catch? Medical Guardian isn’t cheap. Their monthly rates run higher than Bay Alarm Medical or MobileHelp, and fall detection costs extra on every plan. Whether the faster response times justify the price premium is a personal call, and we’ve laid out all the numbers below to help you make it.

MGHome Landline

Starting at $27.95/month, this is Medical Guardian’s most affordable option. It connects to your existing landline and provides up to 1,300 feet of pendant range. A 32-hour backup battery keeps it running during power outages.

MGHome Cellular

The cellular version runs on AT&T’s network for $34.95/month. It extends pendant range to 1,400 feet and adds an LED display on the base unit. No landline required.

MGMini

The MGMini is Medical Guardian’s most affordable mobile system at $36.95/month. It’s a pendant-style device roughly 2 inches tall and 1 inch wide — compact enough for all-day wear. Features include 4G LTE, GPS, Wi-Fi location tracking, and a five-day battery. Caregiver tracking through the MyGuardian app is included at no extra cost, which is unique among Medical Guardian’s lineup.

MGMini Lite

The MGMini Lite is a bracelet-style variation of the MGMini. It weighs just 0.004 pounds — half the MGMini’s weight — with a 36-hour battery. Monthly cost starts at $42.95. Caregiver tracking costs $2.99/month extra (unlike the MGMini, where it’s included).

MGMove Smartwatch

The MGMove is Medical Guardian’s only smartwatch system. In addition to emergency monitoring, it supports step counting, weather updates, medication reminders, and voice texting with caregivers (via the Support Circle app, $5/month extra). It starts at $38.95/month with a $199.95 equipment fee. Battery life is approximately one day.

Purchasing and Cancellation Policy

Medical Guardian systems are available online, by phone, and at retail stores including Walmart. There are no activation, installation, or cancellation fees.

If you want to cancel, call the company and return your equipment. You’ll receive a prorated refund for unused subscription time. Note that cancellation is not available during the first 90 days of service, so you’re committed to at least three months.

Caregiver App

The MyGuardian app (available on iOS, Android, and desktop) provides caregivers with:

  • Real-time device location tracking
  • Battery level monitoring
  • Emergency call notifications
  • Medication reminder setup
  • Billing account management

Caregiver tracking is free with the MGMini. For all other systems, it costs $2.99 per month extra.

Monitoring Quality

Medical Guardian’s monitoring centers hold TMA Five Diamond, ESA Security, FM Approved, and UL Listed certifications. In our testing, agents answered calls in an average of eight seconds — the fastest response time among all medical alert companies we’ve evaluated. Operators can also communicate in multiple languages through a third-party translation service.

Is Medical Guardian Worth It?

Medical Guardian is the right choice if response speed is your top priority. No other company we’ve tested comes close to an 8-second average. The comprehensive MyGuardian app and wide device range (up to 1,400 feet) are additional strengths.

The tradeoffs are real, though. Monthly costs run higher than most competitors, fall detection costs extra on every plan, and the equipment fees are a meaningful upfront investment.

Best for: Users who want the fastest possible emergency response and don’t mind paying a premium for it.

Consider alternatives like Bay Alarm Medical if: Budget is your primary concern — Bay Alarm starts at $24.95/month versus Medical Guardian’s $31.95.

Prices and availability should be verified on the provider’s website.
